Semester Benefit Request for Military Educational Benefits

Please read the following instructions on how to access our forms:

  • Click the link above "Semester Benefit Request"
  • Log in to the DocRoute system with you Jag number and Jagnet password
  • Select Academic Affairs on the lefthand dropdown menu and then Military Services Forms
  • Click Semester Benefit Request blue button
  • Complete the form
  • Remember to click "insert" after every line of text before submitting
  • Use this form for INITIAL semester benefit request. Do not use this form for schedule changes.

This form should be completed only if you need to notify our office of a change to your schedule after your initial semester benefit request. Please follow access instructions above but instead click the "Schedule Change Request" button.

If you are ONLY using the èßäÉçÇø¹ÙÍø GI Dependent's Scholarship, you do not need to complete this request.
